Home Care Services

Continuing Health Care has launched a community care service to meet St Louis Estate residents’ daily care needs. This service comprises a coordinated and planned approach by providing a “whole of service approach” to address current care service gaps. We aim to prevent unnecessary admissions to acute hospitals, and defer if not delay indefinitely admission into resident aged care so that our residents can remain in the comfort of their own home longer.

Our care model is overseen by registered nursing staff who provide on-going assessment, planning and implementation of higher level care needs, including for residents with dementia. Care staff offices and a fully operational consulting room is provided on site at St Louis Estate and is used by visiting Doctors, podiatrists and alternative health practitioners.

Services available include:

- activities of daily living including assistance with washing, showering, mobilisation, dressing, eating and meal provision;

- referral assistance to specialised medical services and appointment scheduling and transport assistance; and

In the future CHC will seek to obtain Community Aged Care Packages and Extended Aged Care at Home (including Dementia) Packages, subsidised by the Federal Government of Australia, so that we can provide a comprehensive and fully integrated care service.
